Fatal AMTRAK train crash blamed on possible gate and or light malfunction

According to Federal Investigators, the gates and warning lights at a railroad crossing weren't functioning properly and may have been turned off when an Amtrak train fatally struck a teacher.

The Federal Railroad Administration’s investigation revealed witness accounts and video footage which has confirmed the active warning system was not working when the train struck and killed the driver of the SUV.
